Early Intervention and Placement Program Overview

The Early Intervention and Placement Program, generously funded by NSW Family and Community Services, is dedicated to providing essential information, support, referrals, and case management services to young individuals aged 12 to 18 years, along with their families residing within the Wollondilly Local Government Area.

At WSNC, our Case Managers employ a strengths-based methodology, working collaboratively with children, young people, and their families to ensure comprehensive support. The case management process includes several key components: Intake, Assessment, Case Planning, and connecting clients to suitable programs and services tailored to their unique needs. Each Case Plan is specifically designed to address the individual’s circumstances, utilizing a holistic approach that considers all aspects of their lives.

We welcome referrals from various sources, including other agencies, government departments, families, and self-referrals. For those seeking additional information or assistance, please do not hesitate to reach out by calling 02 4774 1273, sending a text message to 0456 609 477, or emailing familysupport@wsnc.org.au to obtain our referral form.

Furthermore, we encourage young people to visit the WSNC center and engage with a youth worker. Our dedicated youth workers are available to assist with a variety of important areas, including:

  • Development of essential life skills

  • Enhancing social skills and building healthy relationships

  • Financial management and budgeting guidance

  • Career advice and support

  • Self-help strategies and fostering connections with family and educational resources

We are here to support and empower young individuals as they navigate their challenges and opportunities.
